fix(adoption-insights): show user friendly entity titles in top lists

Description

• The Name column displays metadata.title when it exists. If it is missing or empty, fall back to metadata.name.
• Hovering over the titles shows a tooltip with entityRef | type | description..
• Long titles are truncated with ellipsis and do not break the layout
